Missing of placement request form on SIWES Portal

I have been trying to filling my siwes but I couldn't see the request placement form where to filling the company chosen for SIWES. So I can not further on my portal. I need a solution & resolving to this issue at hand. This particular form is missing on my portal.

I think the authority has added me on siwes portal that is three weeks four days today. But I have been facing setback since the first week, that place to add my siwes record is missed. But sometimes it appears but when I try to add my details no way.
Pls sir or madam succour me to complete my tasks thanks.

Responsibility

Quite unfortunate and some feelings of disappointment that so many complaints and difficulties from students but no single responded from school official.

Why should students be mostly neglected to wander about at most difficulties due to technical problems of the school.

Administrative sometimes at NOUN towards students facing challenges is very very poor.

Someone need to do his/her job as required else we students feel the pains most.

Failure to download Forms

Please why are you making life difficult for we students...? We texted both here and through Email but no response whatsoever. At least this is what we must do and yet you refused to let the students do what they supposed to do. Please find solution to this issue, some were able to do theirs while MOST didn't.
